• Note 1: Much of the writing on the right-hand side of p. 309 is now missing due to a tear along the long side of the page (this does not affect p. 310 to the same extent as the tear occurs in what is the left-hand margin of that page). The tear is small enough, though, to remove only a few letters on each line and we have been able to deduce what most of the missing letters are from their context; they are here supplied within square brackets.
