- Format: Foolscap sheets were folded to make three sets of
four sides, on which the entries were recorded. These sheets were then folded again,
the first and third (i.e. pp. 153-6, 181-4) once vertically and twice
horizontally (in thirds), the second (pp. 177-80) once vertically and once
horizontally (in half). The last side of the third sheet, p. 184, is
blank. Pages 5, 9, 10 and 11 have original pagination.
